Medical Network and Benefit Shifts to Inspect

Medicare Advantage plans change their medical benefits and provider directories every year. Your notice highlights these modifications in an easy-to-read, side-by-side comparison table.

Rising Maximum Out-of-Pocket Limits

Every Medicare Advantage plan includes a Maximum Out-of-Pocket (MOOP) limit for covered medical services. Once you hit this threshold, your plan pays 100 percent of covered medical services for the remainder of the year.

The Centers for Medicare & Medicaid Services (CMS) raised the federal in-network MOOP ceiling to $9,850 for 2027. This marks an increase from the $9,250 cap enforced in 2026.

For Preferred Provider Organization (PPO) plans, the combined in-network and out-of-network ceiling rises to $14,800 in 2027, up from $13,900. Plans use these higher limits to offset rising medical expenditures.

In practice, the median actual in-network MOOP established across Medicare Advantage plans sits at $7,100 for 2027. This represents an increase from the $6,500 median limit observed in 2026.

The CMS-projected average monthly premium for Medicare Advantage drops slightly to $12.00 in 2027 from $14.37 in 2026. Approximately 70 percent of Medicare Advantage plans with drug coverage continue to charge a $0 plan premium.

Remember that you must still pay your standard Original Medicare Part B premium alongside any private plan charge. The Medicare Trustees Report projects the standard Part B premium will rise to $209.50 monthly in 2027, with an annual deductible of $292.

Stricter Rules for Chronic Condition Perks

Many enrollees choose Medicare Advantage plans for supplemental wellness allowances. However, access to Special Supplemental Benefits for the Chronically Ill (SSBCI) faces strict new restrictions in 2027.

Under revised CMS regulations, enrollees can no longer self-certify their chronic health conditions to claim these benefits. Plans must verify objective medical criteria before approving perks like utility support, grocery allowances, or wellness supplies.

If your plan previously gave you monthly debit cards for groceries, check your notice closely. You may need written clinical documentation from your physician to retain those benefits next year.

Electronic Prior Authorization and Carrier Exits

Navigating medical approvals becomes simpler in 2027. Starting January 1, Medicare Advantage plans must process prior authorizations electronically, allowing you to track coverage decisions through your member portal.

Network alterations represent another urgent area to inspect. Insurers frequently drop medical groups, hospital systems, and specialists to negotiate lower operating costs.

Several major carriers have also reduced their geographic footprint for 2027. For instance, Humana discontinued Medicare Advantage plans covering roughly 600,000 members across various counties.

If an insurer exits your service area entirely, they do not send a standard ANOC. Instead, affected enrollees received a formal plan non-renewal notice in early October granting them special rights to choose new coverage.
