New GLP-1 Coverage and Chronic Disease Support Programs
The Medicare GLP-1 Bridge Program for Weight Loss
Medicare is introducing the Medicare GLP-1 Bridge program in 2027 to address weight loss and metabolic health. As detailed in the official Medicare & You handbook, this initiative provides coverage for select Glucagon-Like Peptide-1 therapies.
Qualifying Part D beneficiaries who meet specific clinical criteria can access covered GLP-1 medications for weight loss at a fixed $50 monthly copayment. This program provides affordable treatment options previously excluded from standard Medicare coverage.
Speak with your prescribing doctor to verify whether you qualify under the clinical guidelines of the bridge program. Your physician must submit appropriate documentation showing your medical necessity for weight reduction and health improvement.
The ACCESS Care Model for Chronic Health Conditions
Original Medicare is launching the ACCESS care model in 2027 to modernize chronic disease management. Available at low or no cost, this program supports beneficiaries living with ongoing conditions like hypertension, diabetes, or depression.
The ACCESS framework connects your primary care physician with specialized partner organizations to deliver coordinated home-based support. This team-based approach provides continuous care between your routine doctor appointments.
Enrollees receive personal lifestyle coaching, regular check-in consultations, and advanced medical monitoring tools, including wearable health devices. These connected monitors transmit daily vitals to your clinical team, allowing doctors to adjust treatments proactively.
