Cost-Sharing Plans and High-Deductible Variations

Plan K and Plan L require you to share more costs up front in exchange for significantly lower monthly premiums. These two policies establish an annual out-of-pocket maximum to protect your finances.

For 2026, Plan K caps your annual out-of-pocket spending at $8,000, which increases to $8,360 in 2027 under official guidance from the Centers for Medicare & Medicaid Services (CMS).

Plan L covers 75 percent of covered costs and limits your spending to $4,000 in 2026. This out-of-pocket limit climbs to $4,180 in 2027.

Once you meet your annual limit, Plans K and L pay 100 percent of covered Medicare costs for the remainder of that calendar year.

High-Deductible Plan G and High-Deductible Plan F present another cost-conscious option. You pay Medicare cost-sharing expenses until you satisfy a $2,950 deductible in 2026, which rises to $3,050 in 2027.

Plans C, D, F, G, M, and N cover 80 percent of foreign emergency care during the first 60 days of international travel. This coverage applies after a $250 annual deductible and maintains a $50,000 lifetime benefit cap.
