Frequently Asked Questions About Medicare Dementia Support
Does Medicare pay family caregivers a direct cash salary?
No, Medicare does not provide cash payments, paychecks, or direct subsidies to family caregivers. The $2,625 GUIDE respite allowance pays contracted care agencies directly rather than distributing cash to families.
If you require financial compensation, check your state’s Medicaid program. Many state Medicaid waivers allow self-directed care, permitting qualifying individuals to hire family members as paid caregivers.
Why are Medicare Advantage plan enrollees excluded from the GUIDE model?
CMS designed the GUIDE pilot to test alternative payment models within Original Medicare. Because private insurers administer Medicare Advantage plans, CMS cannot test direct fee-for-service delivery models within commercial networks.
However, some Medicare Advantage plans offer caregiver supports, adult day care allowances, or respite hours. Review your plan’s Evidence of Coverage or contact your insurer directly to discover available dementia benefits.
How do Caregiver Training Services differ from the GUIDE model?
The GUIDE model is a comprehensive pilot program that includes care navigation, 24/7 crisis lines, and funded respite breaks. It is available only through select participating providers to Original Medicare patients.
In contrast, Caregiver Training Services (CTS) are permanent Medicare Part B services available nationwide from enrolled clinicians. CTS focuses on practical skill instruction and requires standard Part B coinsurance payments.
What happens if we do not spend the full $2,625 respite allowance before July 1?
Respite care funds operate on an annual cycle and do not roll over from one performance year to the next. Performance Year 2026 begins on July 1, 2026, and concludes on June 30, 2027.
Any remaining portion of your $2,625 allowance expires at the end of the performance year. On July 1, a fresh respite allowance becomes available, provided your loved one continues meeting eligibility guidelines.
