Respite Care Funding and Cost Protections
Caregiver burnout is a primary reason families place individuals with dementia into residential facilities. To help you recharge, the GUIDE model includes covered respite care services.
To manage emotional fatigue alongside respite breaks, family members can also utilize Medicare coverage for mental health and counseling for professional therapy.
For Performance Year 2026, which runs from July 1, 2026, through June 30, 2027, the respite care allowance is $2,625 per eligible patient. This annual cap increased from $2,500 at the 2024 launch and $2,563 during 2025.
The $2,625 allowance is not a cash payment or reimbursement check. Instead, Medicare pays contracted GUIDE respite providers directly for services rendered.
Your annual respite funds reset every year on July 1. Unused funds do not roll over to the next performance year, so coordinate with your Care Navigator to schedule breaks throughout the cycle.
Eligible Respite Settings and Zero Cost-Sharing
You can use your respite allowance across several settings depending on your loved one’s needs. Options include in-home care aides, licensed adult day health centers, and short-term respite stays in residential facilities.
Best of all, eligible families pay $0 in cost-sharing for covered GUIDE services and respite care within the annual limit. You face no deductibles, copayments, or coinsurance charges for participating in this pilot model.
The table below outlines how the GUIDE model compares with other Medicare caregiver support options.
