New Medicare Prior Authorization Rules: Which Treatments Are Affected

CMS finalized 14 active procedure categories requiring prior authorization before scheduling across six demonstration states.

Specific Treatments Requiring Prior Approval Under WISeR

CMS originally evaluated 17 outpatient services historically linked to high rates of billing fraud or medical overuse. After excluding inpatient procedures like deep brain stimulation from the initial phase, CMS finalized 14 active procedure categories.

If you live in one of the six demonstration states, your physician must navigate authorization before scheduling these specific interventions.

Pain and Spine Interventions

Pain management and spine treatments represent a substantial portion of the targeted procedure list. Epidural steroid injections for chronic back pain require advance review, though facet joint injections remain excluded from WISeR.

Percutaneous vertebral augmentation procedures, frequently performed to stabilize vertebral compression fractures caused by osteoporosis, are also subject to review.

Percutaneous image-guided lumbar decompression, commonly used to relieve pressure from spinal stenosis, requires advance clearance. Outpatient cervical fusion surgery must also undergo prior medical review before doctors proceed.

Orthopedic and Joint Procedures

In orthopedics, the WISeR model focuses on arthroscopic knee procedures. Specifically, arthroscopic lavage and arthroscopic debridement for knee osteoarthritis now require advance authorization.

Federal clinical guidelines have long questioned the long-term therapeutic value of routine knee cleanout surgeries for wear-and-tear arthritis. The WISeR model enforces stricter medical necessity scrutiny on these interventions.

Nerve Stimulators and Neurological Care

CMS also placed several neurological stimulation devices under the pilot umbrella. Permanent spinal cord stimulator implantations for intractable chronic pain require prior approval before surgery.

Other targeted nerve interventions include sacral nerve stimulation for urinary incontinence, phrenic nerve stimulators for breathing dysfunction, and vagus nerve stimulation.

The list also covers hypoglossal nerve stimulation for obstructive sleep apnea. Additionally, surgically induced lesions of nerve tracts, specifically neurolytic destruction of the trigeminal nerve, require prior clearance.

Urological Devices and Advanced Wound Care

In urology, Medicare targets incontinence control devices, specifically mechanical or hydraulic systems designed to treat severe stress urinary incontinence.

Surgical diagnosis and treatment of impotence, specifically the operative insertion of inflatable penile prostheses, also requires advance clearance under WISeR.

Finally, the model covers specialized skin and tissue substitutes. These cellular and tissue-based products are often applied to treat severe diabetic foot ulcers and non-healing venous leg wounds.

If your doctor recommends any of these 14 services in a pilot state, verify that authorization is secured before receiving treatment.
